Outside of the Trust
We know some staff need or prefer to have their vaccinations elsewhere, such as with your local GP.
If you’ve had your flu vaccination outside of the Trust, please let us know so we can update our records. Simply email [email protected] with the date you had the vaccine.
What do I need when I get my flu jab?
When you get your jab, you’ll be asked to fill in a slip (which you can download and pre-full below) and advised to add:
- Your personal details – Including your date of birth
- Your payroll number – Your payroll number can be found by logging in to ESR and is on your payslip as ‘assignment number’. If you are not paid by the trust (e.g resident doctors), you can leave the payroll number blank.
- Your NHS number – If you don’t know it, you can find your NHS number online or on your NHS App.
