How to claim travel costs
When attending your appointment, please speak with a member of the clerical team when you arrive and book in for your appointment.
Please ensure that you bring evidence to show you are receiving one of the above benefits.
We will issue you with a travel reimbursement form to complete and provide details on what the next steps are.
If travelling by bus you will need to show your ticket as evidence of travel, if you have parked in a non-Trust car park you will also need to show evidence of your car parking costs – such as a ticket to claim parking charges.
