Speech and language course booking form

REGISTRATION INFORMATION: Registration information, including venue details and timings, will be sent to the email address supplied to us on the booking form at least seven days prior to the event. It is your responsibility to ensure you provide us with a valid email address and to ensure that you forward information to delegates as necessary. We will not be responsible for the non-arrival of registration information. Any delegate not receiving the registration information by 7 days prior to the event should contact us on 07827 954333. CANCELLATIONS: If you cancel your place at less than 3 weeks’ notice, the full course fee will be charged. If you inform us that you are/were unable to attend due to sickness or other exceptional circumstances we will try to offer you a place on the same course on another date at no extra cost. You may give your place on a course to an alternative delegate at any time at no extra cost, but please inform us of the change. We reserve the right to cancel a course at any time without liability. Such cancellations occur only very rarely. Should we be obliged to cancel a course, we will telephone you on the number provided on this booking form, giving as much notice as possible. Delegates will then be offered an alternative date, a different course, or a refund of the course fee.
