Cardiac Rehabilitation

If you have suffered a heart attack or heart problem, we will make sure you are helped along the way to your recovery. We support all patients with a new diagnosis of coronary heart disease, before and after heart surgery, tests or before you get an internal cardiac device.

Our specialist cardiac rehabilitation team runs a programme for people who have a heart condition to look at their lifestyle and make any necessary changes to avoid further problems. Cardiac rehabilitation is a very important part of your treatment; we work in partnership with you to help restore you back to good health using various programmes including the heart manual, revascularisation plan and the angina plan.

You may hear the doctors and nurses talk about your recovery in phases.

Phase one

You will start the first phase of your cardiac rehabilitation programme in hospital. We know it can be worrying being in hospital, so we want you and your family to receive information, advice and reassurance about your condition during this phase.

Phase two

This is your six weeks recovery period following discharge from hospital. When you leave hospital our community cardiac rehabilitation team will step in to help you.

Phase three

This is a structured exercise and education programme to help reduce your risk of coronary heart disease. Our caring team will work with you through the programme, which is an eight week exercise and education programme, tailored to meet your needs.

Our cardiac nurse specialists work with exercise specialists at a local gym. You will complete gentle exercise, with a warm-up in a safe monitored environment.

Phase four

This stage is about making lifelong changes to further reduce your risk of coronary heart disease.

You will be referred to us by your GP or hospital consultant.

How to make a referral

Referrals can be by letter or hospital discharge summary by post or electronically.
