What is happening?
Patients who need ongoing support often have to provide us with a lot of information so we can make sure their treatment is as effective as possible.
We need to gather information to assess a patient’s current health status before treatment begins as well as examine how they are recovering afterwards.
Traditionally this information is collected during face-to-face appointments. This means the patient has to make extra visits to hospital and it takes up time for the clinician.
Our pilot scheme will offer the opportunity for patients to complete these forms online provided by DrDoctor and save a trip to the hospital.
Who is DrDoctor?
DrDoctor is a Patient Engagement Platform used by over 24 million patients across the UK.
They partner with over 50 NHS Trusts and Health Boards to provide technology that supports patient communications, appointment management and remote care.
How I will be contacted about using the online form?
Your clinical team will advise if you are eligible to be part of the pilot scheme.
When patients are sent a form via DrDoctor messages will come from 07860 039092 and/or email containing a secure link.
Patients are asked to provide their surname, date of birth and post code to access the form.
Is it secure?
Yes. DrDoctor is completely secure.
I would prefer not to use the online form.
