What is residential rehabilitation
To support hospital discharge or to help prevent an admission to hospital you may be admitted to one of the non-chargeable residential rehabilitation units to support in your recovery back to independence.
You might need to be in residential rehabilitation if you are medically fit enough not to be in a hospital bed, but you can’t manage to do everyday tasks at home yet. We can help you to regain physical strength, confidence and functional skills through provision of support up to six weeks.
Residential rehab units across Stockton and Hartlepool are staffed by a variety of health and social care professionals such as Physiotherapists, Occupational Therapists, Nurses, Support workers and Social Workers. These staff all work closely together to develop an individualised plan of support to enable you in your recovery. This can include rehabilitation activates to support you with;
- Improving your mobility
- Dressing
- Getting washed
- Making a meal or hot drink
- Exercise programme to improve strength and flexibility
- Getting in and out of bed
- Accessing the community
During your stay, the team will support you to plan your discharge from the residential setting. This will include engaging with local services with aims to return to your normal levels of independence.
Making a referral
Referrals are through health or social care professionals involved within your current plan of care. Referrals are made to Single Point of Access who coordinate the admissions through discussion with the referring health professional and care home staff.
Our centres
It can house up to 44 people across four separate units, to support residents for up to six weeks.
The centre has a number of facilities to make your temporary stay as comfortable as possible. These include:
- Emergency call systems
- Internet access
- Gardens
- Single level building
- Access to district nursing and community matron services
- Communal lounges/dining rooms on each unit
- Therapy gym
Westview Lodge is a care home that houses up to 74 people. It has a separate rehabilitation unit to support residents for up to six weeks.
The centre has a number of facilities to make your temporary stay as comfortable as possible. These include:

- Wheelchair access and a lift
- Internet access
- Pets by arrangement
- Therapy Gym
- Communal lounge
- Dining room
- Emergency calls systems
