Our dedicated team of district nursing sisters, community staff nurses and health care assistants provide nursing care to patients who are housebound, 24 hours a day, seven days a week in Stockton, Hartlepool and some surrounding areas.
We work closely with your GP practice, social care, local authorities, nursing and healthcare teams, community matrons, hospital services and Macmillan nurses.
We care for people who are over 18 if they are in their own home or a residential home to:
- give medicines by injection
- provide wound care
- provide end of life and palliative care
You will be referred to us by your GP or other health professional.
