What is this leaflet about?
This leaflet aims to provide information about our service for patients and their families.
We are a team of specialist and community healthcare professionals who work with children, young people and their families to provide personalised support. Our goal is to offer practical help and care to everyone who accesses our service.
Who are the members of the team?
Our team includes a range of Nurse Specialist and Children’s Community Nurses who support children and young people with medical conditions such as Diabetes, Epilepsy and Asthma. The team may come to your home, school or see you in North Tees or Hartlepool Out-Patient Department.
What happens next?
If you are referred to our service, a member of the team will contact you to discuss the next steps. They will arrange to visit you at home, in clinic or in the hospital, depending on your needs.
How do we book appointments?
We can offer appointments between 08:00 a.m. to 06:00 p.m. Monday to Friday. We are closed during public holidays.
If the child or young person is unwell and unable to attend the appointment, please provide as much notice as possible by contacting the Children’s Services Administration Hub. Please be aware that missed appointments or visits may result in longer waiting times.

We may not be able to answer your query straight away but aim to respond to you within 48 hours.
If you have concerns about your child’s health please seek medical advice from your child’s GP or NHS 111 (if non-urgent), or emergency services (999).
