Information for patients
This leaflet can be made available in other formats including large print, CD and Braille and in languages other than English, upon request.
After your recent consultation or treatment, you have been placed on a Patient Initiated Follow up (PIFU) pathway. This leaflet will give you more information about this process.
What is Patient Initiated Follow up (PIFU)?
This means we will not make you regular review appointments. Instead you can make an appointment to see your clinician or healthcare team if you need it.
This will strictly be if you have:
- A flare up.
- Symptoms related to your condition that worsen.
- The feeling you are not recovering well.
You can make contact and book a telephone consultation appointment if you have any concerns related to your specific Urological condition or symptoms until:
Date: ………………………………………
What do I need to do to book my appointment?
If you need an appointment, please phone the Urology secretaries on (01642) 383801 Monday to Friday between the hours of 9am to 5pm and leave your contact details.
You will then be given an upcoming appointment which will be by a telephone consultation. Face to face can be booked only if the clinician feels it is necessary.
What happens if I do not ring the hospital?
If you do not ring the hospital within the timescale you have been given, you will be discharged from the hospital and back to the care of your GP or the original referrer.
You can contact them to be referred to us again if needed.
When should I not use this service?
- If your concern is related to a different condition or symptoms.
- If the time period stated above has lapsed, then please see your GP.
- If you need urgent medical advice you should contact your GP or NHS 111.
